Florida Chapter Local Care Services
Until there is a cure, there is care.

The ALS Association Florida Chapter offers comprehensive support and resources for people living with ALS, their caregivers, and loved ones. The goal of local care services is to create an informed, empowered and well-cared for ALS community through providing services and assistance to anyone struggling with issues related to ALS. All services and programs provided by care services are free of charge and are funded solely through fundraising efforts of The ALS Association Florida Chapter.

Clinic Satisfaction Survey

The ALS Association Florida Chapter is interested in learning about your experience with the care and services you have received from the Chapter and the ALS Multidisciplinary Clinics in Florida. Your feedback is important to help us identify opportunities for improvement to serve you better. This short survey will take you approximately 5-10 minutes to complete.
