If your company has an employee matching gift program, you could double the impact of your gift to The ALS Association MN/ND/SD Chapter.

Check with your human resources or community relations department, and ask for the proper form to request that your company match your gift. Some companies match gifts made by retirees and/or spouses.

If your company is eligible, take these steps to request a company match for your donation:

1. First contact your employer. Most likely, your employer will require you to fill out a form documenting the amount of the donation, date given, and the organization it was given to.
2. Your employer may require the following information of The ALS Association, MN/ND/SD Chapter when you fill out this form:

3. After you fill out your employer’s company match form, be sure to indicate the MN/ND/SD Chapter as the recipient of your donation and matching gift. Your employer may contact The ALS Association directly to confirm your donation was received or they may ask you to send us your form.
4. Once The ALS Association confirms your donation, companies usually take 1-6 months to send the matching gift. Once this gift is received, it will be added to the webpage  of the event participant.
